Assumption of Risk/Waiver of Liability
This agreement hereby releases Champions Training Center, its owners, members and instructors from all liability relating from injuries that may occur during the use of or present or future participation in any and all classes, services, equipment, demonstrations or events. By signing below, I responsibility for any injuries incurred, regardless of cause
I hereby acknowledge that I am fully aware of the risks and hazards inherent in the practice of Martial Arts, conditioning and exercise. These risk include but are no limited to sprains, breaks, lacerations, sore muscles, paralysis or death. I swear that I am of sound physical and mental condition, am participationg voluntarily and able to use the facilities of Champions Training Center Additionally, I do not have any condition(s) that will increase my likelihood of experiencing injury while engaging in these activities.
By signing below and participating in I, on behalf of myself, heirs and assigns, acknowledge and represent that I have read this release, understanding it and agree to all conditions.
